Crushing of kidney stones (lithotripsy)

Modern medicine allows crushing kidney stones using various methods. But the most effective is percutaneous laser lithotripsy. In this surgical intervention, an endoscope is used, which is inserted under the skin through a small puncture in the lumbar region. A laser is brought through the channels of the endoscope, which destroys calculi, i.e. stones in the kidneys.

Advantages of percutaneous laser lithotripsy

Although this method of stone crushing is widespread in urology, it is considered a rather complicated type of intervention. Therefore, the key to the success of the operation is the experience and competence of the surgeon.

The main advantages of percutaneous lithotripsy include:

  • high efficiency of destruction of concretions;
  • low trauma: a single puncture is made on the skin, and other organs of the urinary system are not damaged;
  • a short period of hospitalization and subsequent recovery.

Indications for crushing kidney stones

The formation of kidney stones is a characteristic symptom of urolithiasis. And while small stones can still come out on their own, medium and large ones definitely need medical attention. The main reason for seeking it, as a rule, is severe, acute pain in the lumbar or groin area – renal colic.

Percutaneous laser lithotripsy is used for single or multiple stones. The main indications for this type of surgery include:

  • large calculi (larger than 3 cm);
  • some types of stones, including coral-like and cystine stones;
  • impossibility of using other methods, including remote crushing or using a flexible endoscope;
  • stones in an abnormal kidney or the only one left.

Contraindications to percutaneous lithotripsy

This technique is not used in case of blood clotting disorders and urinary tract infections. It is temporarily contraindicated to crush kidney stones with percutaneous lithotripsy in case of:

  • pregnancy;
  • skin lesions in the lumbar region where the puncture is performed;
  • pathologies that interfere with access to the kidney.

How to prepare for lithotripsy?

Beforehand, urologists at the Diyevo Center prescribe a set of preoperative tests, including:

  • laboratory tests of blood and urine;
  • fluorography;
  • Pelvic ultrasonography;
  • electrocardiography;
  • consultation with an anesthesiologist;
  • other examinations if indicated (for example, women are also recommended to visit a gynecologist).

Performing laser lithotripsy

The operation is performed using anesthesia. To access the kidney with stones, a puncture is performed in the lumbar region. Then an endoscope is inserted, and through it, laser equipment (lithotriptor), which crushes the calculi into small fragments. At the end of the procedure, drainage is placed (if necessary), and after its removal, a sterile dressing is applied. The duration of the operation depends on the number and size of stones to be crushed.

The patient remains under medical supervision for some time (at least a day). Recovery may involve taking medications, dieting, and limiting physical activity.

Pyelolithotripsy is a method of crushing a kidney stone in the pelvis. The differences between the methods of crushing a stone in the renal pelvis are in the access of the instrument to the stone.

There can be access:

  • Transurethral – through the urethra – bladder – ureter – without a puncture!
  • Percutaneous – through the skin of the lumbar region to the kidney.

We use a holmium laser lithotriptor as a stone crushing device.
