Laser dissection of urethral narrowing

Stricture (narrowing) of the urethra is a pathological narrowing of the internal lumen of the urethra, which leads to urinary disorders of varying severity.
Symptoms of urethral narrowing
The causes of strictures include either a congenital defect or an acquired one. In the second case, it can be the result of injuries, wounds to the pelvis and pelvic organs. Also, narrowing can occur as a result of inflammatory processes or surgical interventions on the genitourinary system.
You should consult a doctor if you notice the following symptoms:
- intermittent urination;
- longer urination;
- the appearance of a weakened urine stream;
- defecation occurs when the abdominal press is strained;
- feeling of incomplete bladder emptying.
Our experience shows that men can walk around for years with similar symptoms that reduce their quality of life, and use drug treatment, which is completely ineffective for this pathology.
Diagnosis of urethral narrowing
There are very frequent cases when urethral stricture does not manifest itself and is often disguised as such diagnoses as prostatic hyperplasia or chronic prostatitis. To detect the presence of stricture, it is necessary to perform special diagnostics.
To establish an accurate diagnosis, it is recommended to undergo the following examinations:
- Taking a general urine and blood test;
- Urethrography is an X-ray method;
- Pelvic ultrasonography;
- Uroflowmetry (physical characteristics of urine flow);
- Urethrocystography (if necessary).
After receiving the results of the examination, this diagnosis is confirmed or refuted, as well as the cause of the disease, the site of narrowing and the presence of complications are identified.
Treatment of urethral stricture (narrowing)
Urologists at the DIEVO clinic in Kyiv perform urethroscopy for all men who come to the clinic with urinary disorders and do not have prostate hyperplasia (adenoma), which allows to establish the presence or absence of urethral narrowing with 100% certainty.
There are different methods of treatment: voiding (mechanical dilation) of the urethra, endoscopic optical laser urethrotomy, urethral stent placement, open urethral plastic surgery – the doctor chooses the type of treatment taking into account the individual characteristics of the body and the degree of the disease.
Optical laser urethrotomy is most often used to treat urethral strictures.
This is an operation that results in transurethral (inside the urethra) opening of the scar ring to healthy tissue to increase the urethral lumen. The purpose of the operation is to create a sufficient urethral lumen for free urination.
Optical urethrotomy
Stages of the operation:
- examination of the stricture area and assessment of the nature and involvement of the surrounding tissue in the scarring process;
- insertion of a flexible metal conductor through the stricture into the bladder under the control of ultrasound scanning and fluoroscopy;
- laser dissection of scar tissue in parallel to the conductor under optical control, followed by vaporization of the scar tissue;
- installation of a temporary urethral catheter for 1-7 days.
