Narrowing of the ureter (hydronephrosis)

Ureteral narrowing is a congenital or acquired pathology that occurs due to narrowing of the transition site of the renal pelvis into the ureter.

Congenital pathology usually manifests itself by the age of 30. Acquired pathology develops as a result of trauma, tumors, or urolithiasis.

A severe stricture leads to a significant obstruction of the urine outflow from the kidney, and the kidney function suffers, which can lead to diseases such as hydronephrosis, urolithiasis, and pyelonephritis. As a result of these pathological changes, renal failure can develop, which can be acute or chronic.

Symptoms of ureteral narrowing

A pronounced impairment of the free outflow of urine from the kidney and the development of various pathological processes against this background: hydronephrosis, pyelonephritis, urolithiasis – in case of bilateral damage – chronic renal failure.

Patients experience acute or dull pain in the lumbar region, fever, signs of intoxication, increased fatigue, nausea, vomiting, and convulsions. In such patients, the amount of urine produced decreases. The urine that is still passed may be cloudy and have a pungent unpleasant odor.

Diagnosis of ureteral stricture

  • Ultrasonography of the kidneys

  • CT scan of the kidneys
  • MRI

The diagnosis of ureteral narrowing is an absolute indication for surgical treatment.

Treatment

If the damage to the renal tissue is minimal, then various reconstructive surgeries of the corresponding ureter are performed to eliminate the narrowing of the ducts and restore the free outflow of urine from the pelvic lochanteric apparatus of the kidneys.

In case of severe upper urinary tract lesions and the development of renal failure, open or puncture nephrostomy is used.

Also, sometimes endoureteral adhesion dissection with stent placement, bougie and balloon dilatation of the narrowed ureter are performed, but they do not give a lasting effect and can lead to even greater complications.
